How To Request Refills
On-Line
- Beneath
the "Prescription Refill Requests" title on the main
page, click on the pharmacy where you plan
to pick-up your refill.
- Enter
the date and time you would like to pick up your refill(s).
Please allow 24 hours for your request to be processed.
- Enter
your name, birth date, insurance plan, and allergies in the
spaces provided.
- Enter
the six- or seven-digit prescription number (located in the upper left
corner of your prescription label) for each refill you are
requesting. Include all zeros; do not include numbers after the
dash at the end.
Note: If your label indicated 'No Refills Remaining" at the
bottom left of your label, please contact your physician to
request a new prescription.
- If
you do not have your prescription numbers, please type the
medication name(s) for the refills you wish to request.
- Enter
the city where you live as well as daytime and evening phone
numbers, and your e-mail address.
- Click
on "Submit."
