Consultation/Refer a Patient

We encourage direct communication before consultations to ensure our clinical understanding, streamline any necessary testing, and expedite referrals to specialists at UI Hospitals and Clinics, if appropriate.

Patient medical records will be needed at the time of consultation.

Physicians referring a patient to UI Family Care can call UI Consult at 800-322-8442 or fill out an online patient referral form.
